Licensed Practical Nurse/LPN - Home Care

POSITION SUMMARY: Responsible for providing nursing services to clients in their home. This position will work closely with the RN to perform duties as delegated.

Responsibilities include but are not limited to:

• The primary function of this position is to complete medication orders, re-fills and overall medication oversight for the clients in our care.
• Position will include supervisory visits, chart audits and general nursing practices as required and delegated by the RN.
• This position will communicate with the RN to perform duties as delegated but will need to be self-sufficient and provide leadership in absence of nursing staff.

QUALIFICATIONS:

• Home care experience preferred, new graduates considered.
• Associate's Degree is required, with Licensure as a License Practical Nurse in the State of Minnesota
• Candidates must have strong customer service skills with the ability to prioritize.
• Excellent computer skills to navigate electronic documentation.
• Candidate will be subject to a criminal background check.
• Candidate must have reliable transportation and an unrestricted motor vehicle record.

PHYSICAL ESSENTIAL FUNCTIONS:

• Ability to sit, stand, speak, and walk for long periods of time
• Ability to bend and stoop using good body mechanics
• Frequent lifting, carrying, pushing and pulling up to 30 lbs.
• Occasional lifting, carrying, pushing and pulling 75-100 lbs.
• Some exposure to infectious waste, toxic chemicals, needles/body fluids, mechanical hazards and driving
