Burnsville Carefree Assisted Living Facility, 

Performs and/or ensures delivery of programs and services for all residents in the Memory Care Unit; supervises staff to ensure the safety and well being of all residents. Routinely and effectively communicates with resident families/guardians regarding safety and well being of residents.

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

  • Will perform and/or ensure delivery of services for all residents.
  • Will provide Health Services Manager and/or Executive Director input and will follow/or appointed designee to follow the directives for care/service needs
  • Reports changes in resident status, falls and/or incidents to Health Service Manager and/or Executive Director
  • Enters weekly a note summary in medical record for each resident. Reports changes in condition to family/guardian.
  • Ensures (by leading or delegating) activity and life enrichment programs are provided on a daily basis to meet resident needs.
  • Identifies and implements resident specific interventions to decrease behaviors.
  • Assist Health Services Manager and/or Executive Director in providing orientation to new employees.
  • Creates and maintains resident care communication system to ensure quality care on each shift.
  • Monitors all common and private areas on a regular basis ensuring all are clean and orderly. Reports concerns to Maintenance Supervisor and/or Executive Director.
  • Supervises and assists during resident meals and monitors for appropriate diets.
  • Performs duties as weekend "manager on duty" on a rotating basis.

Qualifications:

- At least two years of experience working with seniors and at least one year working with senior who have memory loss (such as Alzheimer's or Dementia).

- Ability to handle multiple priorities

- Ability to delegate assignments to the appropriate individuals based on their skills, roles and interests

 

- Possess written and verbal skills for effective communication and the ability to facilitate small group presentations

- Competent in organizational, time management skills

- Demonstrates good judgment, problem solving and decision making skills

-  Associate's Degree is required, with Licensure as a License Practical Nurse in the State of Minnesota
