RN Clinical Manager

Our established homecare agency in Rochester, MN is accepting applications for a RN, Clinical Manager to join our team!  

Position is full-time. Working primarily daytime hours with flexibility to meet the changing needs of our residents and staff during evening and weekend hours.  Position includes a shared on-call responsibility. 

POSITION SUMMARY: The Clinical Manager provides oversight of all patient care, services, and personnel. The oversight includes making patients and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care.

• Responsible for implementing the nursing process, including assessment, diagnosis, planning, goal setting, implementation and evaluation, in the delivery of professional and paraprofessional services to home care clients.
• Coordinate/complete the admission paperwork, explaining the care plan and client/family responsibilities.
• Supervise and coordinate the retrieval and maintenance of appropriate, accurate and complete documentation of information on clients and staff under your supervision according to agency policy.
• Oversee clinical operations within the established operating budget in an effort to achieve the organization's operating and financial objectives.

QUALIFICATIONS: 

• Current licensure as a Registered Nurse in the State of Minnesota.
• Minimum of three years' experience in nursing, preferably in an Assisted Living/Home Care setting.

PHYSICAL ESSENTIAL FUNCTIONS:

• Ability to sit, stand, speak, and walk for long periods of time
• Ability to bend and stoop using good body mechanics
• Frequent lifting, carrying, pushing and pulling up to 30 lbs.
• Occasional lifting, carrying, pushing and pulling 75-100 lbs.
• Some exposure to infectious waste, toxic chemicals, needles/body fluids, and mechanical hazards
